SUMMARY: model 114 array controller upgrade

I was getting the following errors:

These failures made it look like I had 6 dead disks (from the Veritas
perspective). I broke the mirrors on the volumes and was able to get
the server and file systems functional until I got the new FC/OM cards.
I replaced the fiber cable and both FC/OM (fiber channel optical modules).
1 in the array and 1 in the server. This fixed the problem. I did not have
to worry about the worldwide name because I did not replace the controller
board itself.
