problem E 450

We have an E-450 Server with 12X18GB internal disks, Veritas Volume
Manager 3.1.1 (with Veritas File System) and Solaris 8 installed by Jan
2001 with NO patches ever installed thereafter.
All of a sudden we started to get the following messages and the big
data/application volume becomes unavailable. After rebooting it works
fine for a day or two. We've already exchaged a bad mirrored BOOT disk,
but still have the same thing. We don't think it could be a physical
problem with disks, but maybe a O.S. or a SCSI controller (PCI)
malfunction.
